To update your producer information, please email fairplan@nypiua.com with your change and include any pertinent attachments:

• For an expired license, please attach a copy of your active NY State Producer's or Broker's License.
• For changes to your name, address or tax information please attach a new W-9, which is available at Home | Internal Revenue Service
• For changes to ACH information, please attach a copy of the Producer Registration (signed section D) along with a copy of your voided check.

Also include your Producer ID, Name, and Contact Information to help expedite the request.
